Replacement Photo Request
The Cal Poly ID photo upload option is primarily for the first printed PolyCard, though we can replace a current picture with an uploaded photo. See the information below:
Details
- The photo must meet the Photo Requirements listed on the upload page.
- The portal photo (students only) will not get refreshed until a replacement PolyCard is printed.
- If this is not a defective ID card issue a $5 fee will be required to replace your PolyCard.
Request Process
- Complete the webform below for replacement consideration.
- Upload the replacement ID photo. ***Be sure to go back to the Submit ID Photo page and complete the upload process.
