### **To request a new or replacement Cal Poly Badge:**

1. Verify the recipient has [uploaded a photo](https://calpoly.atlassian.net/wiki/spaces/CPKB/pages/2425541).
2. Submit a [Badge Request Form](https://calpoly.atlassian.net/servicedesk/customer/portal/2/group/12/create/67).
