If you have Microsoft Forms that you would like to migrate, follow these steps:

> ℹ️ ***NOTE:**** You will only have ****30 days**** post-migration to migrate forms.*

> ### **Export responses from the source tenant            **
> 
> **For each source form:**
> 
> 1. Sign in to the **CSUM Office365**.
> 2. Go to **Microsoft Forms**.
> 3. Open the form.
> 4. Go to **Responses** or **View responses**.
> 5. Select **Open results in Excel**.
> 6. Save the Excel file in your migration archive.
> 
> > ℹ️ Name it clearly, for example: FormName_SourceTenant_Responses_YYYY-MM-DD.xlsx
>

>

> 
> ### **Copy the Form as a Template**
> 
> **For each form:**
> 
> 1. Open the form in the **CSUM Office 365**.
> 2. Select the **ellipsis (…)** or **More options**.
> 3. Choose **Collaborate or Duplicate**.
> 4. Find **Share as a template**.
> 5. Copy the **template URL**.
> 
>  
>

> 
> ### **Duplicate the Form in the Destination Tenant**
> 
> 1. Open a browser session signed in to the **CalPoly Office365**.
> 2. Paste the template URL copied from the **CSUM Office 365.**
> 3. Select **Duplicate it**.
> 4. Rename the copied form as needed.
> 5. Confirm the questions, sections, branching, theme, quiz settings, and required fields copied correctly.
> 
> <span style="color: #0747a6">*The duplicated form is a *</span><span style="color: #0747a6">***new form***</span><span style="color: #0747a6">* in the new tenant. It will not contain the old responses inside the Forms response dashboard*</span>

> ### **Move the Duplicated Form to the Correct Group, if Needed**
> 
> **If the form should be owned by a team or department:**
> 
> 1. In the CalPoly Office365, open **Forms**.
> 2. Go to **All My Forms**.
> 3. Find the **duplicated form**.
> 4. Select **More options**.
> 5. Select **Move**.
> 6. Choose the destination **Microsoft 365 Group**.
> 7. Confirm the form appears under that group.
